Simply fill out this form and submit it, listen to 104.5 for your cue to call in, or mail a postcard to WQKT/WKVX, 186 S. Hillcrest Dr., Wooster, OH, 44691. Mail and online entries must be received by noon, September 2, 2010 in order to be entered into the drawing. Drawing will be held at noon on September 3, 2010 at the offices of WQKT/WKVX.

You must be 18 or older to enter and you must show proof of identity in order to secure the prize packet.

We will make reasonable effort to contact you if you are a winner. If we do not receive a response by you by September 8, 2010, either by phone or email, you will forfeit the prize and we will draw another winner. We will award 10 sets of 2 passes and tickets.

The winner’s acceptance of prizes provides permission for WQKT/WKVX to use name and likeness of the winner in all Wooster Radio publications including www.wqkt.com.

One (1) entry per person is permitted. Contest winners are restricted to only one winner per household every 30 days and must be 18 years or older. Winner must show proof of identity in order to secure the prize packet.

Neither WQKT/WKVX, nor its employees, may be held liable for any costs, damage, injury, or any other claims incurred as a result of usage of any winners once possession has been made by winner.

Management and employees, and their families, of WQKT/WKVX are prohibited from winning any prizes awarded by Wooster Radio.
